GP Forms & Letters We Can and Cannot Provide 

Requesting a Letter from Your GP

Patients request letters from their GP for a wide range of reasons, and our doctors are frequently asked to provide private letters or “to whom it may concern” letters for third parties.
The standard fee for a basic letter starts from £30, although more complex letters or reports may incur a higher charge.

Timescales

Private letters and reports do not take priority over clinical work.
Please allow up to 28 days for your request to be completed. 

Content

We cannot accept requests specifying that “the doctor must write these exact words.”
GPs will only write what they know and what can be supported by your medical records.
The clinician’s opinion and the content of the letter are final, and amendments cannot be requested.

GPs also reserve the right to decline your request or use alternative wording if they feel it is necessary or clinically appropriate.

Letters We Do Not Provide

•    Passport or driving licence endorsements

•    Seatbelt exemption certificates

•    Character references for employment

•    Letters for not attending or missing a court appearance due to illness or for probation purposes. We can only issue these if we receive a direct  court order instructing us to do so.

•    Letters for not attending school, missed or deferred exams, coursework, schooling, or university due illness.

•    School toilet pass.

•    Fitness to participate in high risk activities such as parachuting, diving, marathons, or extreme sports.

•    PIP application or appeal letters.
We will complete a report if the Department for Work and Pensions (DWP) requests one directly.

•    Occupational Health reports 
These must be carried out by a qualified Occupational Health professional. None of our GPs or clinical staff hold this qualification.

•    Blue Badge - We do not provide letters to support blue badge applications. We will only provide a medical summary to support your application.  Please allow up to 28 days for your request to be completed.

•    Home Office – proof of registration: A patient summary will be provided in the first instance. If this is not sufficient, charges will apply for any additional letters.

Reports and complex letters are charged according to the time taken to assess the patient and produce the report.  This charge is based on the amount of time taken by the GP to complete the work, from a rate of £160 per hour. The charge will reflect level of GP and administrative input. The minimum fee is £30.
